Kerryn thrives on working closely with participants in their own homes, building trusted relationships with individuals and their families. She is passionate about helping people access the supports and services they need to live safely, independently, and with confidence. Known for her strong communication and collaborative approach, Kerryn works effectively with a person’s entire care team to ensure all aspects of their support are aligned and working well together.
Outside of work, Kerryn is a proud mum of three girls and plays an active role in her local netball club where she coaches a team, and still finds time to play the game herself.
She’s also a passionate Collingwood supporter—but we don’t hold that against her.


With over 30 years of nursing experience across major public hospitals, Jan brings a deep understanding of healthcare and a compassionate, person-centred approach to her role. She has worked in a wide range of clinical settings, with particular strength in Rehabilitation—supporting patients recovering from TAC incidents, orthopaedic procedures, and neurological or cardiology conditions. Her work in multidisciplinary teams has shaped her adaptable and collaborative style.

For over 13 years, Jodie managed aged care homes, with a strong focus on quality and risk management—always advocating for the wellbeing and dignity of those in her care.
Now, with over a year of experience as a Support Coordinator, Jodie continues to bring her deep healthcare expertise and unwavering passion for helping others into the NDIS space. She’s embraced the role with dedication and empathy, making a meaningful difference in the lives of participants every day.


Mandy brings 26 years of nursing experience to her role, having started her career in the cardiology unit at the Alfred Hospital. For the past 22 years, she has focused her expertise on aged care community nursing, excelling in roles such as field nurse, manager, and clinical educator.
A valued member of our team for over a year, Mandy combines her extensive clinical knowledge with a genuine passion for supporting individuals with diverse health needs. She is dedicated to empowering clients to live safely and independently within their homes and communities.

Emily began her career in disability support as a Support Worker while completing her nursing diploma. She then spent five years at Epworth Private as a Specialist Cardiac Nurse, including working on the infectious disease ward during the pandemic. Following this intense period, Emily chose to refocus her career on community health, where her passion truly shines.
Known for her high attention to detail and thorough approach, Emily communicates regularly and effectively with the entire care team to ensure seamless coordination of supports. She is dedicated to helping participants achieve their NDIS goals through a holistic and collaborative approach, empowering individuals to live their lives on their own terms.
Outside of work, Emily is a devoted St Kilda supporter, attending as many games as possible. She also enjoys walking her brown spoodle, Albert, and hosting dinner parties for friends.


With a background as a Registered Nurse and Midwife, and hands-on experience as a Disability Support Worker, Beth brings both clinical experience and compassion to her role as a Support Coordinator.
Passionate about empowering individuals to live with dignity, independence, and joy, Beth champions person-centred care that truly reflects each participant’s unique goals and values.

Outside of her professional role, Jen balances a busy family life with three very active teenagers. She values spending time with family and friends and enjoys travelling — or at least planning the next holiday.


Jack brings four years of dedicated experience as a Support Worker, having joined Support Chain in 2021. He has developed a versatile skill set working with a broad spectrum of clients—from supporting children with ASD to assisting adults with complex needs, including brain injuries, substance misuse, and diverse social backgrounds.
Known for his genuine commitment to empowering others, Jack thrives on helping individuals of all ages pursue their goals and improve their quality of life. His recent completion of an Advanced Diploma in Community Services further enhances his ability to provide informed, compassionate support.
Outside of work, Jack is actively involved in his community and enjoys a range of outdoor and social activities, including 4×4 driving, AFL, camping, golf, and spending quality time with friends.


Katrina brought her compassionate spirit and wealth of experience straight to Support Chain when she joined in 2021, drawing on over 25 years in the health and community sectors—as well as her own lived experience supporting family and close friends with disabilities.
Known for her kind and considerate nature, Katrina is deeply passionate about advocating for her clients’ independence, wellbeing, and overall quality of life. She creates a friendly, supportive atmosphere where participants feel heard and empowered.
Katrina takes pride in helping NDIS participants manage their daily tasks and confidently work towards their short- and long-term goals, always tailoring her support to meet their unique needs.


Cameron—known as Cam to friends and colleagues—is passionate about making a positive difference in people’s lives. With four years of experience supporting individuals with physical and intellectual disabilities, as well as clients recovering from brain injuries, Cam has been an integral part of our team since he started his career in support work.
Cam is dedicated to helping participants discover activities and opportunities that promote independence and enhance their overall wellbeing. His enthusiasm and commitment continue to make him a valuable member of our team as he grows in this rewarding field.
Outside of work, Cam enjoys playing cricket and football, staying active and connected with his community.


Kaitlyn is a qualified nurse who currently works at an Urgent Care Clinic and brings valuable experience from her time working in an Emergency Department. Alongside her clinical role, she has also been a dedicated Disability Support Worker and a valued member of our team for over five years.
With a broad background in the health sector, Kaitlyn is passionate about improving the lives of others and takes great pride in helping her clients achieve their personal goals.

Before stepping into her new role as an NDIS Support Coordinator, Bec worked as a Support Worker since 2020 and spent several years at a Speech Pathology and Occupational Therapy clinic, working in Client Connections and as a Therapy Assistant Coordinator, where she developed a deep understanding of client-centred care and the importance of collaboration across disciplines.
Bec has always been passionate about helping others – she finds purpose in supporting participants to navigate the NDIS, build their capacity and achieve their personal goals. She enjoys connecting with the people and families she helps.

Now transitioning from Plan Management, Bron has taken on the role of General Manager at Support Chain. In this position, she plays a key role in guiding our team to continue delivering exceptional service and positive outcomes for our participants.

You’ll work with a designated support coordinator to assist you one on one to help achieve your goals. Think of your coordinator as a new friend, helping you live your life to the fullest.
There’s no one-size-fits-all approach to our your NDIS plan. NDIS will provide you with a tailored plan based on your individual goals, and we will help you to implement your supports.
Your support coordinator will work with you to optimise your plan’s funding, giving you equal access to more services and better opportunities.

You’ll come into our office (or we’ll come to you) for a meet and greet, followed by a strategy session for your support plan.

Once you’re happy with your plan, your support coordinator will work with you to connect you to the right services and support.
